One of my friend who was asking for Indian railway reservation form, was not able to go till railway station and bring one back. He was also asking for how he can get it online and then what should be fill in those blank areas. So I searched around internet and find the reservation form (attached with this post). Lets see it first. It looks like this.
What I advised him that he should first saw this on his hard disk and then print it out. This way it would look good.

I also told him to check pnr status of his ticket after booking it from the counter to know exact seat availability information.
